What is Included in Documenting Your Work as a Community Health Worker?

Behind the scenes, a Community Health Worker is required to document a variety of activities and information, which ultimately improves program outcomes and organizational goals. The Documenting Your Work as a Community Health Worker session provides insight into the “why” behind the sometimes burdensome and mundane task of documenting CHW work. The objectives of this session are to:

  • Describe the role of documenting your work
  • Identify and demonstrate appropriate guidelines for documentation of Community Health Worker activities

This training session  on-site  or .

This training session lasts 2 hours and can be provided as a live, fully interactive virtual training for $639 or on-site at the location of your choice for $2,459* including all travel costs.

This session can be completed individually or as a full Level I training. If you would like to mix and match sessions to create your own training package, please contact us for additional information.

*Includes all travel costs for a training location within the continental United States. Note: travel costs will be prorated if multiple on-site training sessions are purchased for the same location and time period.
