What Is Included in Case Management?

Case Management is one of the roles Community Health Workers can play in the course of their work. The Case Management for Community Health Workers session will allow participants to grasp the process of case management, deliver effective care over time, and support clients in achieving their goals. The objectives of this session are to:

  • Understand the role of a Community Health Worker in case management
  • Apply case management skills developing client goals and action plan

This training session lasts 2 hours and can be provided as a live, fully interactive virtual training for $639 or on-site at the location of your choice for $2,459* including all travel costs.

This session can be completed individually or as a full Level II training. If you would like to mix and match sessions to create your own training package, please contact us for additional information.

*Includes all travel costs for a training location within the continental United States. Note: travel costs will be prorated if multiple on-site training sessions are purchased for the same location and time period.
