What Is Included in Case Management and Community Health Workers: Key Factors?

Successful case management in a Community Health Worker program requires the support of a supervisor knowledgeable about the unique issues encountered in the field. The Case Management and Community Health Workers: Key Factors session reviews the role of Community Health Workers in case management and identifies strategies to ensure success in their roles. The objectives of this session are to:

  • Understand the role of a Community Health Worker in case management
  • Discuss strategies for supporting the case management function

This training session lasts 2 hours and can be provided as a live, fully interactive virtual training for $639 or on-site at the location of your choice for $2,459* including all travel costs.

This session can be completed individually or as a full Level II training. If you would like to mix and match sessions to create your own training package, please contact us for additional information.
