Currently, there is a strong movement for Health Centers and organizations to adopt the Patient-Centered Medical Home (PCMH) model to improve care delivery. At the same time, the field of Community Health Workers has emerged as an effective and recognized practice in health care. Community Health Workers can be an exciting and effective incorporation to both newly emerging and already established PCMHs. The following provides a brief summary of how Community Health Workers can fit into PCMHs. While the examples given here describe PCMHs focused on serving migratory and seasonal agricultural workers, the benefits and implications are applicable to a variety of Health Centers or PCMHs serving other underserved communities.
