I hereby grant MHP Salud permission to use my name and likeness to accompany quotes and/or testimonials, to take photos and/or videos, and/or other print and/or digital media (“media”) of me, and to share these in any and all publications, including web- and print-based publications, without limitation to promote the work of MHP Salud and/or its affiliates and partners, without payment, other compensation of any type, and consideration.

I understand and agree that all media files noted above that contain my likeness and/or image will become the property of MHP Salud and will not be returned, nor will I be given or have the right to a copy. I hereby irrevocably authorize MHP Salud to edit, alter, copy, exhibit, publish and/or distribute these media files for the purpose of promoting MHP Salud or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product wherein my likeness appears.

I hereby hold harmless, release and forever discharge MHP Salud and its affiliates from all claims, liabilities, obligations, demands and causes of action of any nature or kind, known or unknown, which I or any other person acting on my behalf have or may have by reason of this authorization.
