Community Initiatives
Throughout South Texas and Central Florida, our Community Health Workers help families and individuals improve their health and well-being. We understand everyone is in a different place in their journey toward well-being … and we meet them where they are, literally and figuratively, to help them achieve positive health and well-being.
Improving Access to Health Care
Many communities we serve face significant barriers to health care, as unique challenges – from transportation to language to system complexity – make it difficult to access even the most basic health services. We provide personalized support to:
Understand, apply for and secure health coverage through Medicaid, CHIP or the federal Health Care Marketplace
Help older adults understand, apply for and secure health coverage, nutrition and housing resources, and access local resources for health and well-being
Help older adults access, understand and use technology to navigate health coverage and services, and to maintain connections with loved ones
Improving Child Development and Well-Being
Parenting is hard … especially without a close support system. Many families we serve live in extremely rural or isolated communities and face significant social stressors – housing instability, limited education and/or incomes, transportation and language barriers, lack of access to health services, mental health challenges and more.
Partnering with parents to support positive child health, development and well-being, our Community Health Workers:
Educate parents about child development and their role as their children’s first teacher
Connect families with health care, housing, baby essentials and more
Support mental health for new parents
Help families reach personal goals for education and employment
Provide opportunities for peer support and life-skills trainings
Promoting Healthy Decisions in Youth
Youth and young adults face unique challenges as they navigate peer pressure and health risks. Our Community Health Workers build trust with youth to provide honest, judgment-free education around:
Substance misuse prevention
HIV/STI prevention
Safe relationships and informed choices
Healthy decision-making
Access to testing and health care services
How We Do It
The heart of MHP Salud is in the community. Because our Community Health Workers live in the communities they serve, they bring deep understanding, connection and trust to every interaction, building authentic relationships that make a difference.
All of our work in the community is in Spanish and English so we can best serve our neighbors.
