Mr. Ernesto, who watched his family members fall ill to chronic disease, was terrified of encountering the same fate. Knowing genetics weren’t on his side, he reached out to MHP Salud for support to find resources for nutrition and access health insurance he knew would be critical (given his family history). After receiving the health insurance he desperately needed, Mr. Ernesto learned he was diabetic — and his glucose levels were off the charts. Scared and confused, he reached out to MHP Salud again, joined our nutrition and physical activity educational program, found connections to health resources and – within months – was able to successfully manage his diabetes. As he shares, “My Community Health Worker is more than my support – she’s my advocate.”
