Serving Our Communities
Our health outcome program consists of several CHW-led initiatives that target various health and social issues such as access to health care, chronic disease control and management, healthy relationships, aging, nutrition, and more.
More than 200,000 individuals reached and 25,000 served through our health outcome programs.
Our CHW-led programs have demonstrated a positive return-on-investment (ROI) of $1.09-3.16 for every $1 invested.
Nearly 2 out of every 3 participants served by a CHW report at least one Social Determinant of Health need such as assistance with food, housing, and transportation being met.
In Texas, which holds the highest uninsured rate in the nation, over 5000 individuals, as of 2017, have been enrolled in a Qualified Health Plan through our Navigator program.
Individuals who received health education and support from CHWs doubled the amount of daily physical activity, consumption of fruits and vegetables; and significantly decreased consumption of saturated fats and sodium.*
MHP Salud CHWs provide nearly 1000 resource referrals per year to program participants for services such as food assistance, medical services, mental health services, and more.
Advancing the CHW Profession
Throughout our history, we’ve been dedicated to supporting organizations in their efforts to improve the productivity and performance of their new or existing CHW programs. Today we continue to collaborate with national health organizations and associations to develop training and resources and provide technical assistance centered around the CHW profession.
In 2021, we supported over 1000 organizations with CHW related activities across 48 states and 2 U.S. territories.

We’ve delivered our L.E.A.D. curriculum through our CHW training to over 70 organizations. Our curriculum aligns with the standards of the C3 Project Core Competencies for Community Health Workers, as well as many state certification programs.
Over 95% passing rate for individuals who participate in our foundational level CHW training (read more about CHW Certification requirements: Map & Directory of CHW State Information – MHP Salud).

More than 1,500 hours of technical assistance provided to organizations with CHW programs across the nation within the last 5 years.

Just in the last year, our resources specifically designed to support CHWs and organizations with CHW programs have been downloaded over 3000 times.

As of 2021, our Management Based Objective program has supported over 700 health centers with CHW related activities.
