We Measure Our Success by the Impact We Have on Our Communities

Our programs provide health education, case management, and application assistance to families and individuals in underserved communities. Led by Community Health Workers, these programs help remove barriers that prevent access to care and ensure that our communities receive the essential services needed to improve health outcomes.

Our health education program for SNAP-eligible families served over 10,000 adults and their children in Texas’ Rio Grande Valley.

Our Navigator program enrolled over 6,000 individuals in a Qualified Health Plan in Texas, the state with the highest uninsured rate in the nation.

Positive Return on Investment

Our CHW-led programs have demonstrated a positive return on investment (ROI) of $1.09-3.16 for every $1 invested.  Positive ROI results from Community Health Worker-led programs prove their success as an effective means to improve the quality of healthcare services while controlling health spending.

Advancing the CHW Profession

Throughout our history, we’ve been dedicated to supporting organizations in their efforts to improve the productivity and performance of their CHW programs. Today, we continue to support health organizations and associations with training, resources, and technical assistance centered around the CHW profession.

We have delivered our L.E.A.D. training to over 80 organizations, training approximately 600 CHWs, CHW supervisors, and CHW allies since L.E.A.D.’s inception in 2018.
