Better Health Outcomes for Our Community
Our Community Health Worker initiatives provide health education, case management, and application assistance to members of their communities. CHWs lead outreach and education activities directly in the neighborhoods where underserved individuals live, work, and play. By removing barriers that prevent access to care, we ensure that our communities receive the services that are essential to improving health outcomes. Although our initiatives address a variety of issues, they all have the same mission:

MHP Salud bases all of its work on close collaboration with CHWs who improve the health of their communities with community-based health education and activities. Since 2017, our initiatives have enrolled over 15,000 participants across the United States and conducted outreach to over 150,000 individuals.
Life Stages
Our Life Stages initiative focuses on the health of individuals throughout all stages of life. Projects within this initiative address perinatal health, aging in place, reproductive health, and other similar health topics.
Healthy Relationships
Our Healthy Relationship initiative provides individuals with skillsets to promote positive relationships and healthy communications, including relationships between parents and children or partners/spouses.
Healthy Living
Our Healthy Living initiative provides support and education to promote good health. Projects within this initiative reinforce healthy behaviors like nutrition and physical activity and provide chronic disease prevention or management support among other things.
Connect to Care
The goal of this initiative is to improve and strengthen healthcare networks and access to quality care. CHWs assist the community in accessing health insurance and other public and social service programs.
National Programs
Training, technical assistance, and resources designed to help organizations reach their goals during the planning, implementation, and evaluation of new or existing Community Health Worker programs.
