Perinatal education, outreach and home visiting services to Latino families.

What we do:

The mission of the Desde el Primer Momento is to reduce infant mortality among Hispanic families in Lenawee County, Michigan. In order to do so, this program provides two pillars of support to the community. The first pillar is to provide education on healthy pregnancies and births to community members through education sessions, presentations, and outreach. The second pillar is to provide direct support to pregnant moms and their families through home visits.

 

How our approach works:

A trained Community Health Worker (CHW) works closely with community partners to provide evidence-based, culturally-competent education on healthy pregnancies to the community. Additionally, the CHW provides direct support to pregnant mothers and their families through a series of home visits throughout her pregnancy and up to 3 months postpartum. The home visits are directed by the mother’s goals for her pregnancy and birth.

 

Goals:

  • Provide perinatal education home visits to 20 pregnant woman in Lenawee County.
  • Provide perinatal education and information to 500 individuals in Lenawee County.
  • Establish partnerships with 3 community agencies in Lenawee County.

The Desde el Primer Momento program serves the communities of Lenawee County, Michigan.
