We’re currently seeking a Community Health Worker Apprentice to join our team. Motivated, energetic, organized, and focused individuals are encouraged to apply. Please see the full CHW Apprentice Job Description. Successful candidates will:

• Attend community events or health fairs to understand community issues and build relationships with community members.
• Assist in the community with providing application assistance and referrals, virtually or in person, as needed and follow up to help ensure services are received.
• Conduct outreach to increase family health awareness and activities.
• Identify and/or contact members of high-risk or otherwise targeted groups such as members of minority populations, low-income populations, or pregnant women. Advise clients or community groups on improving general health, such as diet or exercise.
• Understand and be part of the community being served.
• Have experience in community work, education, health care, or related field, preferred.
• Have the ability to read, write, and speak both Spanish and English; fluency in spoken Spanish required.
• Possess Information Technology skills, including the ability to use laptops, smartphones, internet/online application systems, Microsoft Office, etc.

• MHP Salud covers a portion of health care premiums for employee-only, employee-plus-1, and employee-plus-family plans. We pay 100% of employees’ dental and vision plans, with the option to add coverage for dependents.
• We provide employer-paid life insurance and offer you the ability to add more for yourself and your spouse and children and carry excellent employer-paid long-term disability insurance for our employees.
• You are immediately vested in our 401K retirement plan with an employer contribution of 3% of your salary and an additional match up to 2%.

Salary Range: $16.50 to start, $18.00 after 6 months, $20.50 at apprenticeship completion.
