Addressing Diabetes

Community Health Workers’ unique ability to build strong relationships with the families in their communities allows them to effectively address challenges individuals face when trying to manage chronic conditions such as diabetes. Research has shown that CHW programs influence positive behavior changes in participants like healthy eating, physical activity, and timely disease screening. Additionally, CHWs understanding of their community’s language and cultural norms puts them in the best position to provide health information and facilitate health education sessions. This workshop will provide team members with practical tips, resources, and information to implement a CHW-led diabetes program.

Overview

This 4 hour workshop, was developed based on experiences and outcomes from over 35 years of Community Health Worker-led health initiative programs. CHWs in these programs provide health education, connect individuals to health care, promote healthy eating, encourage exercise, and help families address social needs that can impact health. In particular, our programs have provided pathways to healthy living for individuals at risk for or diagnosed with diabetes by promoting activities which helped them increase knowledge of their health conditions, specifically, understanding A1C levels and blood pressure.

The diabetes workshop can be offered online or in-person at a site of your choosing. An expert MHP Salud employee will guide you through a series of team-based exercises, including brainstorming, organizational assessments, and other collaborative activities. The workshop is most successful when a variety of staff participate, including CHWs, supervisors, and even C-suite professionals. This approach allows participants to hear and incorporate multiple perspectives into their program plan and gain a better understanding of how every piece of their organization will be involved in their CHW program.

By the end of the session, you will have the tools that your organization needs to design and implement a successful CHW-led diabetes program in your community. The team will also develop a plan and identify assets and challenges during the implementation process.

Pricing Breakdown

Each workshop lasts 4 hours and can be provided as a live, fully interactive virtual training for $889 with a maximum of 10 participants, or on-site at the location of your choice for $2,709* including all travel costs, with a maximum of 20 participants.

*Includes all travel costs for a training location within the continental United States. Note: travel costs will be prorated if multiple on-site training sessions are purchased for the same location and time period.
