The Community Health Worker Program Implementation Guide

MHP Salud has been implementing Community Health Worker programs since 1985 to address health-related issues among the Latino community. Programs such as these recruit and train local women and men to be Community Health Workers or in Spanish, Promotores or Promotoras de Salud.

Community Health Workers improve the coordination and information flow between community residents and community resources, and empower the Community Health Worker–and through them, their communities–to take more control in improving their own health. The Community Health Worker Program Implementation Guide is meant to be a resource for any agency that has an interest in creating a Community Health Worker program. While the information in this Guide is largely based off of MHP Salud’s expertise implementing Community Health Worker programs with Latino populations, it can be adapted to fit the needs of any program.

To provide a comprehensive overview of a successful Community Health Worker program, this Guide includes some key information, including:

This resource is formatted as a program manual

  • The Program Coordinator
  • Networking, Collaborations, and Forming Partnerships
  • Program Implementation
