Hispanics are the largest minority in the United States and have higher rates of diabetes in both adults and children as compared to other racial/ethnic groups. 1 Latinos are among the fastest-growing groups in the United States and, yet, as a whole have low access to medical care and poor general health partly due to sociocultural factors related to economic status.

It includes a space to keep a record of changes in weight, BMI and A1C across multiple dates, and it includes tips for controlling symptoms of diabetes in both English and Spanish. The tool is meant to be printed and kept in a purse or wallet.
MHP Salud Announces Partnership to Expand Chronic Care Management in South Texas with CHWs
MHP Salud is participating in a project to expand proactive, chronic care management to residents of Texas’ Rio Grande Valley who are either uninsured or covered by Medicaid. The project involves close collaboration among a team of community partners representing the clinical, academic, and nonprofit sectors. MHP’s role is to hire and manage Promotores(as) to provide participants with health education, navigation, and other related services.
