As has been widely reported in the media, the risk of death and severe illness from COVID-19 increases with age. MHP Salud Announces Partnership to Expand Chronic Care Management in South Texas with CHWs
MHP Salud is participating in a project to expand proactive, chronic care management to residents of Texas’ Rio Grande Valley who are either uninsured or covered by Medicaid. The project involves close collaboration among a team of community partners representing the clinical, academic, and nonprofit sectors. MHP’s role is to hire and manage Promotores(as) to provide participants with health education, navigation, and other related services.
