Community Health Workers (CHWs) have been used effectively to help improve a variety of health outcomes. CHWs can provide community members with social support and education in a culturally appropriate manner, facilitate access to care, and build upon existing community networks for greater impact. This makes them uniquely qualified to be champions for individuals who have or are at risk for diabetes.
