Community Health Worker programs address many topics in different settings. This allows for potential funding opportunities from a variety of sources. We’ve developed these resources to help organizations identify appropriate grant opportunities, find alternative forms of charitable funding and to explore financing and funding models for CHWs programs. All resources are available for free and licensed to be distributed for educational use.

This budget is presented as a sample only, based on a year-round program with four full-time Community Health Workers. It is provided as a starting point to give you an idea of the line items and categories of expenses that you will want to consider in planning your budget.
This example of a grant proposal or narrative for a CHW program is based on standard sections found in requests for proposals from the federal government. Organizations can use this sample as a model of how to describe community health needs, explain a CHW program, and write objectives.
