Over 30 million people in the United States (or 9.1% of the population) have Diabetes. About 7 million of those with diabetes have not been formally diagnosed. Community Health Workers and Diabetes Interventions: A Resource for Program Managers and Administrators is a resource for Community Health Worker (CHW) Program Managers and Administrators that explains the positive impact that CHW-led diabetes interventions can positively impact individual patients as well as organizations and communities.
