Community Health Workers and the Social Determinants of Health

According to the World Health Organization (WHO), the social determinants of health are the “conditions in which people are born, grow, live, work and age.” These circumstances, shaped by the distribution of money, power, and resources, are primarily responsible for health inequities.

Today, global and national organizations and individuals have developed a vast new literature on the social determinants of health, including research on their importance, how to understand and define them, and the development of tools to identify them and address them.

This brief presents general information on social determinants of health, available tools for data collection on SDOH, and the role that Community Health Workers (CHW) may play in implementing SDOH programs. It is available for free to our resource portfolio members.
