Nine Tips for Integrating CHWs into Health Center Teams
Community Health Workers (CHWs), known as Promotoras in Spanish-speaking communities, live in and speak the language of the community they serve. CHWs expertise in the culture of the community puts them in a unique position to contribute to Health Center care teams. This connection to their community can also help a care team better understand the context behind a patient’s condition, leading to the development of more effective care plans. Although CHWs can be an asset to any care team, to flourish in this role they need Health Center support.
We’ve developed this resource, available for download with a free membership, that pinpoints nine tips that will help your Health Center integrate CHWs into your care team. You can also download the PDF version below.
