Su Corazon, Su Vida/ Your Heart, Your Life (SCSV/YHYL) Guided Implementation Project is an organizational effort aimed at highlighting the efficacy of CHW programs in addressing hypertension within Health Centers (HCs). This project was designed to provide HC Staff (Community Health Workers, Community Health Worker Supervisors, and other Health Professionals) support, training, and technical assistance through the process of implementing a hypertension CHW program among their patients.
